Ordering Instructions

Option #1: Download the order form (Msword format).    E-mail the completed form to: salesdepartment.nutritionink@verizon.net (please rename the file, use your facility name followed by the word "order" as the file name).  Mail check or money order payable to Nutrition Ink.   Reference the order number from the fax/e-mail confirmation on the check/money order.  The form is also available in PDF format.

Orders will not be processed until payment is received.

Option #2: Credit Card, or Pay Pal account through the online ordering process.

NOTE: pay pal has the option for setting up an account - this is NOT necessary and can be by-passed.
